This has happened about 3 or 4 times. When I get my phone from charging, my selected home screen image has changed to the default home screen. Why does it keep doing this?

I just got the s6 last weekend and so far so good.
My only problem right now is that I'm not a Fan of the image that is displayed when you bring up the lock screen.
Is there a way that you can change that image to something else more my style?
I just bought a new Samsung galaxy s5 and the screen has words and a background image. Its like the samsung logo and galaxy logo and some more that I cant make out. You can see it on the screen and esp when you use the camera.

It is happening especially if im calling an automated system because it makes me navigate back to the phone screen then have to pull up keypad again. Im not sure if I changed a setting by mistake.
