I have never set up a passcode on my iphone 4S but when I try to reset my phone it is asking for a 4 digit passcode. Is there a generic code? Or how do I retrieve an existing/forgotten passcode?

Recently my mum's iPhone 5 stopped working and she tried to take it back to Apple for repair but they said they couldn't fix it until she gave them her login details however she has forgotten everything but still remembers the security questions. What can she do to get her login details back so she can fix her phone? I was thinking we go to the Apple Store and give them some details about the account such as the address used, security questions and date of birth used on the account but i'm not sure if that works. Is it possible?

We WANT to restore it and not have any old data. She purchased an ATT Go phone sim card, so that she can use it with the phone. it works, says it's activated, but still won't do anything without the passcode. Isn't there a hardware reset or something?
